Registration

Two easy ways to register:

1. Online (only if paying by credit card): Please complete the fields in the online registration form and click “submit”; your registration will then be forwarded to the Conference office. Once your payment has been processed, the Conference office will send your confirmation by email.

1. By fax or mail: Complete the PDF registration form and fax or mail it to the Conference office with your payment (see contact information below). Make cheque or money order payable to “Canadian Institute for Health Information” and reference “CIHI/WHO-FIC Network Annual Meeting 2010.” Once your payment has been processed, the Conference office will send your confirmation by email.

Registration forms must be received no later than September24^th, 2010. Contact the Conference office at conferences@cihi.ca if you have any questions relating to payment processes. 

Note: once registration is received a confirmation of attendance will be e-mailed to participants.

CONFERENCE FEES 

Cancellation/Refund Policy

Written cancellations received by September 30, 2010, will be refunded (less the administration fee of $100, plus applicable taxes). Cancellations received after this date will not be refunded. Delegates may substitute an alternate attendee.
