Registration

Two easy ways to register:

  1. Online (only if paying by credit card): Please complete the fields in the online registration form and click “submit”; your registration will then be forwarded to the conference office. Once your payment has been processed, the conference office will send you confirmation by email.


  2. By fax or mail: Complete the PDF registration form and fax or mail it to the conference office with your payment (see contact information below). Make cheque or money order payable to “Canadian Institute for Health Information” and reference “Data Users Conference 2010.” Once your payment has been processed, the conference office will send you confirmation by email.

Registration forms must be received no later than August 31, 2010. Contact the conference office at conferences@cihi.ca if you have any questions relating to payment processes.

Note: Starting July 1st a 13% HST replacing the 5% GST will be added to the total.

Cancellation/Refund Policy
Written cancellations received by August 31, 2010, will be refunded (less the administration fee of $100, plus applicable taxes). Cancellations received after that date will not be refunded. Delegates may substitute an alternate attendee.

Note: Your registration information will not be processed until all applicable payments are received. Registration is limited; register early to ensure a confirmed spot at the conference. On-site registration will be available only if the conference is not sold out.
