Cancellation/Refund Policy:
- Written cancellations received by November 12, 2007, will be refunded (less the administration fee of $100, plus applicable taxes). Cancellations received after that date will not be refunded. Delegates may substitute an alternate attendee.
- Written cancellations for the social event received by November 12, 2007, will be refunded in full. Cancellations received after that date will not be refunded. Delegates may substitute an alternate attendee.

Registration
Two easy ways to register:
- Online (to be completed only if paying by credit card): Please complete the fields in the online registration form and click “submit;” your registration will then be forwarded to the conference office. Once your payment has been processed, the conference office will send you a confirmation by email.
- By fax or mail: Complete the PDF registration form and fax or mail it to the conference office with your payment (see contact information). Make cheque or money order payable to the Canadian Institute for Health Information and reference HHR Conference 2007. Once your payment has been processed, the conference office will send you a confirmation by email.
